What are you doing with the PGD & PGC pins? I had a system than would would not start just like this and in the end it was something attached to one of those, putting it into some sort of programming state. Can't remember how I fixed it... possibly pull ups... I wish I could remember the precise cause and fix - it wasn't that long ago. going senile.
